I, the undersigned do hereby acknowledge that all information submitted is accurate as at the date of submission and that any intentional misleading information submitted will render my registration null and void. I also acknowledge that my submitted registration is non-refundable and covers the period May 1^st of the year of submission to April 30^th of the subsequent year.

I agree that my application upon submission is a contractual agreement with the Jamaica Amateur Body Building and Fitness Association (hereafter, the “JABBFA”; the “Association”) and I commit to the following:

1. All titles won at the National Championships or any other Championships hosted by the Association are the Association’s property. Permission for commercial use of such titles by the athlete who earns such designation requires the athlete to seek written consent from JABBFA for such use.
2. I agree to compete in a Drug-Free environment complicit with World Anti-Doping Agency’s requirements and the requirements of the local Anti-doping regulators, JADCo (Jamaica Anti-Doping Commission).
3. I will not conduct myself in any manner or be affiliated with any action that will bring the Association into disrepute or tarnish the image of the Association and the sport of Body Building and Fitness.

I do hereby grant to the JABBFA and its agents and affiliated companies the irrevocable right and license to use, in the manner agreed, my picture, name and biographical material concerning me, and all rights in and to any video tape, motion picture and/or still photographs made by the Association of my likeness, poses, acts and appearances, and any sound recordings made by the Association in any such article, video tape and/or motion picture as well as any quotes I may give without additional compensation to me, in any manner or medium, whether now known or hereafter developed, in connection with the sport of Body Building and Fitness or the work of the Association, all rights therein and all elements and editions/versions thereof (including, without limitation, any abbreviated or modified versions thereof) and in connection with the advertising and promotion thereof, of the sport of Body Building and Fitness by the Association.

I, the undersigned, further release the JABBFA, its partners, officers, directors, employees, agents, successors, assigns and licensees from any and all claims that I have or might have by virtue of or arising out of the use of the Production, including without limitation, any claim for defamation, slander or invasion of privacy or infringement of rights of publicity.

The foregoing permission and release are given for the Association’s benefit and for the benefit of each of its successors, licensees and assigns and any persons, films, or corporations which, through the Association, shall have the right to promote, distribute and/or exhibit such Production, as agreed by the Association.  Without in any way limiting the foregoing, I acknowledge and agree that the JABBFA will have no obligation to use the images/recordings (or any portion thereof), or to complete, distribute, exhibit or exploit the any of the aforementioned containing myself.
